Anorexia: Not Just a Problem for Girls

Most people do not associate anorexia with boys. According to the ANRED (Anorexia Nervosa and Related Eating Disorders, Inc.) website, many surveys indicate that up to 10% of the people with anorexia and bulimia are males.

Because this is not something normally associated with boys, diagnosis can be more difficult. This particular boy has always been thin but his mother was also very thin as a child so no one thought much of it. Until one day when he was running in a track meet and could barely finish the race. His lips turned blue and he said he felt as though he could not breath. His mother, who happens to be the most competent nurse I know, was obviously very concerned and immediately scheduled an appointment to see a cardiologist.

During his echocardiogram, the doctor saw some irregularities and, at first, thought he may have a hole in his heart. Further investigation led the doctors to believe that perhaps there was pressure being put on his heart by his sternum and his rib cage, which are sunken in. This led to the recommendation that he undergo surgery to repair this, which would entail breaking all of his ribs and shifting the position of his sternum with rods. Thankfully, my friend did not blindly accept this diagnosis but continued to seek other opinions. It was finally determined that the problem was an eating disorder. This bright, overachieving child was starving himself to death and the physical problems were all related to his lack of caloric intake.

Was he worried about being fat? No. Being very intelligent but also very impressionable, he was taught in school that fat and sugar are bad for your body. His concern was not about his body image but about taking proper care of himself. He was convinced because he heard that fat and sugar are bad that he should not put anything bad into his body. As a result, he started to check the labels of all foods and not only watch his fat and sugar intake but his calorie intake as well. He also became obsessed with exercise at this time. He was, and in fact still is, convinced that restricting his diet is what is best for his body.

Unfortunately, because he had been restricting his diet long enough for his body to show physical symptoms of decline, he is in for an uphill battle. Early detection of the problem can be very important. There are certain characteristics common to people with eating disorders and recognizing them can put you on alert to look for other symptoms.

According to Helpguide.org, there are biological causes of anorexia. Research shows that a genetic predisposition to eating disorders can exist in families. If a sibling has anorexia, a person is 10 to 20 times more likely than the general population to develop the problem themselves. In addition, scientist have shown that people with anorexia have higher levels of cortisol, the brain hormone associated with stress, and lower levels of serotonin and norepinephrine, which are associated with feelings of well-being.

Obviously there are also psychological red flags as well. Most anorexic's tend to be overachievers and perfectionists. They excel in everything they do and focus on pleasing others. Despite the outward appearance of having everything under control, inside they often feel helpless and inadequate. They are very critical of themselves and feel if they are not perfect, they are a failure.

Specific behaviors to look for include eating only a very restricted diet and avoidance of "bad" foods such as carbohydrates or foods high in fat, the obsessive reading of food labels and the measuring or weighing of portions, lying about eating, e.g. stating they had a big lunch or their stomach hurts, development of strange food rituals such as using only a specific plate or smelling their food and offering to cook for the family so they can control what they are consuming.

So what do you do if you suspect someone you are close to has developed an eating disorder? A trip to the doctor to confirm the diagnosis is the first step. Despite the possible serious physical effects of anorexia, it is a treatable condition. The treatment is often a lengthy and difficult process that involves not only the family but medical professionals, as well.

My friend's son sees a dietician on a weekly basis and is told exactly how many calories he must eat per day. He complies on most days but not without an argument. He must constantly be monitored by his parents which puts a lot of stress on the family. He is also in counseling to attempt to get to the root of the problem because despite his intelligence, there is nothing logical about his current behavior and attitude toward food.

Although this seems like a daunting problem, the worst thing that you can do is to ignore it. Please do not think it is a phase or something that your child will outgrow. Without proper treatment, 20% of people with anorexia will die but with treatment that number drops to 2% to 3%.
